The sending rescue listed above hereby transfers full ownership and custody of the dog identified in this form to The Golden State Great Pyrenees Rescue and Sanctuary. Upon signing this agreement:

• The sending rescue relinquishes all rights, claims, and interests to the dog.

• The Golden State Great Pyrenees Rescue and Sanctuary assumes all responsibility for the dog’s care, training, adoption, and/or future transfer in accordance with its policies.

• The sending rescue affirms that to the best of its knowledge, all information provided about the dog is accurate and complete.

• All known medical records, vaccination history, and microchip documentation are included with the transfer.

The sending rescue agrees to indentify and hold harmless The Golden State Great Pyrenees Rescue and Sanctuary, its representatives, volunteers, and affiliates from any claims, demands, or liabilities arising from the dog’s actions or history prior to the date of transfer.
